Selecting your doctor Selecting your doctor is one of your most important healthcare decisions. You can find more information about the doctors you know and trust under Find A Doctor or you can call (701) 530-6000, toll-free 1-800-472-2113.
Scheduling your appointment You can make an appointment by stopping in at any of our locations, or by calling (701) 530-6001, toll-free 1-855-530-6001. You can also request an appointment online through our Patient Portal by clicking the My MDC Chart button on the top of the page.
When scheduling an appointment, please provide us with the general reason for your medical visit and the following information:- Full name (married and maiden)
- Phone number (home and work)
- Date of birth
- Mailing address
- Billing party's name
This information will be requested whenever an appointment is scheduled to ensure speed and accuracy in identifying your medical record.
If your appointment involves special services such as an insurance physical, a Workforce Safety and Insurance (WSI) examination, Veteran's Administration examination, services provided through Social Services or the Public Health Service, please inform the receptionist so that these services can be properly arranged.
If you have been referred to the clinic by another doctor, please let us know so that we may keep the referring doctor informed.
Canceling an appointment If you must cancel your appointment or will be arriving late, please call us as soon as possible so that another patient may be served during your scheduled time. We strive to reserve adequate time for each patient to ensure quality medical care and minimal waiting time.
Follow-up visits If you need a follow-up evaluation or treatment, please stop at the reception desk on your way out to schedule your next appointment.
TODAY Clinic For your convenience, we offer the TODAY Clinic with no appointment necessary. The TODAY Clinic is generally staffed by doctors of the Family Medicine department and other specialists in Pediatric & Adolescent Medicine and Internal Medicine. The TODAY Clinic is open at the 9th & Rosser location during these hours:- Monday - Friday, 8 am - 8 pm
- Saturday, Sunday & holidays, 9 am - 1 pm
The TODAY Clinic is open all holidays except Easter Sunday, Thanksgiving and Christmas Day. Patients at the TODAY Clinic are typically seen on a first-come first-served basis and rotated among the doctors on duty. To reach the TODAY Clinic, call (701) 530-6200, toll-free 1-800-472-2113.
Emergencies For a life-threatening emergency, call 911 or go to the St. Alexius Emergency & Trauma Center at 10th Street and Rosser Avenue.
Smoking Smoking is prohibited in all areas of the clinic and on clinic grounds.
Parking Ample and convenient parking is available at all Mid Dakota Clinic locations. Spaces for individuals with mobility handicaps are designated in each parking lot.
Wheelchairs Wheelchairs are available for your use in the front main lobby of each clinic. If you use a Mid Dakota Clinic wheelchair to transport a patient to the hospital or adjacent clinic, please return the chair to Mid Dakota Clinic or notify us of where you are leaving the chair by calling (701) 530-6000, toll-free 1-800-472-2113.
Registration On your first visit to the clinic, you will be asked to provide new patient information via a Patient Information Record form. This information will be used for establishing your electronic medical record and assigning a patient identification number. After your initial record has been established, please notify the clinic personnel of any changes in name, address, insurance or billing information. To expedite the check-in process, please bring your insurance card.
Delays We understand that your time is valuable, and we do our best to stay on schedule. However, medical emergencies may arise which might delay your appointment. Should such an emergency occur, we will do our best to keep you informed about the projected length of delay.
Lost and found Please report lost and found items to the main lobby reception desk. The clinic is not responsible for the loss of any personal items. Prescription refills Patients sometimes ask why the doctor may want to see them before authorizing a prescription refill. Some medications may have side effects that can affect your health. Your doctor may want to examine you or conduct tests to ensure that your medicine is appropriate for continued use. Your doctor or nurse can give you more information on the procedures to follow for your refills.
Calling your doctor Our nurses and other staff who answer your phone calls are trained to ask and answer pertinent questions. If necessary, they can arrange for you to talk with your doctor or assist with making an appointment. When talking with our staff, please have paper and pen handy for instructions.
Sometimes, the doctor or nurse is not available to talk with you immediately. Please leave a message following the recorded instructions, and a healthcare professional will return your call as soon as possible.
Your medical record Your medical record is valuable because it provides a continuous history of your medical care. Your doctor may request that you transfer your previous medical record to Mid Dakota Clinic. The information in your record is confidential and cannot be released without your written consent.
Your hospital Mid Dakota Clinic doctors generally serve only on the staff of St. Alexius Medical Center.
